Insurance
Though Associated Orthopedics accepts many insurance carriers, it is important that you contact your insurance company to verify if we are an in-network provider. We currently participate in several managed care and PPO plans. While we do try to stay versed on the policies and procedures of each plan, individual benefit plans may vary. If you are a member of a PPO, HMO or other form of managed care, some requirements must be met. You may have to have a referral, a second surgical opinion or pre-certification for certain tests or procedures. Ultimately, it is your responsibility to ensure that the requirements of your plan have been met. All applicable co-pays and charges for non-covered services are due at the time of service.
Medicare
Associated Orthopedics routinely accepts Medicare assignments, and will submit Medicare claims on your behalf. We will need a copy of your Medicare card to verify your eligibility and process your claim. Please note that if a Medicare assignment is accepted, you are responsible for 20% of the approved amount, as well as any deductible not met by Medicare.
Medicaid
If you are eligible for Illinois Department of Public Aid, we will need a copy of your IDPA card. Payment of your co-pay is due at the time of service. Please note that Medicaid has payment coverage limitations for certain services and procedures.
Payment
Associated Orthopedics is aware that many medical expenses are unexpected and can cause financial hardship. If you have no insurance, or if full payment upon receipt of the balance is a problem for you, please contact us as soon as possible. We accept major credit cards and personal checks.
Worker’s Comp
Worker’s Compensation is an employer-provided insurance that covers an employee’s on-the-job injury. The physicians and staff at Associated Orthopedics are experts in work-related injuries and evaluations, and will provide the necessary communication and coordination regarding treatment and work status to employers, case managers and claims adjusters.
At the current time we do not bill for auto accidents or third party liabilities. Your account will be set up as Self Pay and payment will be expected at the time of service.

Your First Visit
Please bring any X-rays or other imaging studies pertaining to your orthopedic condition with you to your first appointment. This allows your provider to conduct a comprehensive evaluation of your condition.
Complete all sections of the patient registration/health history form, and return it prior to your first appointment. If have not received these form you may download them here.
Have your insurance cards and a photo identification card with you. If your insurance is an HMO or other managed care, you must obtain your referral authorization from your primary care physician prior to your first visit.
